Documents Needed To Apply (Copies Required):

Household Verification

  • Copy of lease or mortgage
  • Most recent gas and electric bills

Proof of Income

  • Social Security letter
  • Child support documentation
  • Alimony documentation
  • Unemployment documentation
  • Disability letter
  • Last 2 pay stubs (if paid biweekly)
  • Last 4 pay stubs (if paid weekly)
  • Survivor benefits

Identification

  • Identification cards for ALL household members (including school ID)
  • Social security cards for everyone in household
  • Birth certificate for anyone age 1 or younger
