Special Child Health Services

Special Child Health Services provides Case Management services to families of children with special health care needs. We make referrals to and/or assist families to access state, federal and community resources including, but not limited to:

Case Management is a voluntary, free service provided to children with special needs.

Who is Eligible?
Children age birth to 21 years, who are Salem County residents and have special health care needs. Children with “special needs” include those with:

  • Long-term medical conditions
  • Physical disabilities
  • Birth defects
  • Developmental disabilities
  • Traumatic Brain Injury

The Special Child Health Services Program, together with individual County Board of Chosen Freeholders, funds case management units in each of New Jersey’s 21 counties. This case management unit is available to work with you to promote family-centered, community-based care and coordinate services delivery for your child.
